REQUIREMENTS FOR ADMISSION:

Must be a holder of a Bachelor's degree in Food Science & Technology and/or related disciplines

Duly accomplished Application Form (Form 1) submitted with the following:

• Cover letter, signifying intent and degree sought
• Curriculum vitae
• Original or Certified True Copy of Official Transcript of Records (with General Weighted Average (GWA) stated on the TOR; or a Certification of GWA
• Two sealed letters of recommendation (Form 2) from former professors or supervisors
• Non-refundable fee of Php 200.00
