I. PRE-ENROLLMENT PHYSICAL EXAMINATION OF INCOMING UP MINDANAO STUDENTS
As an incoming first year, the University requires you to undergo a Physical Examination. This will help us know your health status, so we would be guided on how to help you stay fit and healthy here in UP Mindanao.
The PHYSICAL and DENTAL EXAMINATION, AND THE LABORATORY WORK-UPS (which includes Urinalysis, Fecalysis, Complete Blood Count or CBC,and Chest X-ray) shall be performed at the Southern Philippines Medical Center (SPMC) - HEALTH SERVICES AND SPECIALTY CLINIC building.
The Physical and Dental Examination period : June 3 - 28, 2019
June 3 - 7 & June 10 - 14, 2019 - Within Davao City
June 17 - 21 & June 24- 28, 2019 - Outside Davao City
Please provide a photocopy of your admission notice, to be submitted to SPMC during the Physical and Dental Examination.
Please bring with you the following during your scheduled Physical Examination:
1. Urine and Stool specimens.
A.Collect sufficient amounts of stool and urine.
B.Place your urine specimens (at least 4-ml) in a sterilized bottle container/vial or "garapa".
C.Place your stool specimen on a sterilized bottle container/vial or "garapa".
D.Stool and urine must be examined within one hour after collection.
2.No payment needed for Physical and Dental Examination and Laboratory work-ups mentioned above.
Note : If the physician requires prescription and additional laboratory test, expenses incurred must be borne by the student.
Steps to be followed during the Physical Examination are posted in the Hospital to guide you. You are expected to follow the steps accordingly.
Submit your Health Folder (provided by the Hospital) upon enrollment where Clearance Slips will be issued to you. The University Nurse will accommodate you at the UP Mindanao Clinic located at Elias B. Lopez Hall.

II. DORMITORY ACCOMMODATION
There is only one Residence Dormitory inside the campus that caters primarily to first year students. It is divided into two areas - one for men and the other for women. Each room can accommodate a maximum of four (4) persons. It is provided with basic furnishings such as study tables and chairs, bed mattresses and lockers. Bath and comfort rooms are in common area. Due to limited space, priority is given to those who are not from Davao City and those from areas in Davao City that are too far from the campus. In this regard, reservations have to be made early to give us ample time to evaluate and determine who can be accommodated.
For those who wish to make reservation, just accomplish the form provided herewith and send to the address indicated or you may also e-mail your reservation at . The said form must be received not later than June 30, 2019 to give enough time to notify applicants if they can be accommodated. Otherwise they may be referred to private dormitories or lodging houses nearby. Kindly notify us for any cancellation of reservation not later than July 15, 2019, so that we can give your slot to others. Reservations after the due date will be accepted but there is no assurance that they can be accommodated. Monthly rental is P500.00 per person excluding electricity charges. We require two (2) months advance (P1 ,000.00) for dorm fee and two (2) months for appliances before checking in. Room assignments will be known upon check-in period and it is done in random order. Bringing of appliances is highly discouraged. Check-in date will be announced later. Qualified applicants are advised to bring the following aside from their personal items:
1.Beddings (bed sheet, blanket, and pillow)
2.Pail or bucket with dipper
3.Padlock and small chain (for individual locker)
4.Broom, trash bag, feather duster, rugs (for room cleaning)
5. Electric Fan
6. Extension Wire (not sub-standard)
